Coastal Bend College (CBC) now utilizes Rank One Sport for record keeping, injury reports and treatment reports. Starting with the 2022/2023 season you will now complete these forms online. The physical and medical history forms must still be completed on paper and returned to the Athletic Trainer either by scanning and emailing me at [email protected] or sending in the original physical paper. No pictures or screenshots of your medical history or physical forms will be allowed.

Below you will find the directions on how to use Rank One Sports to fill out your required Athletic Forms:

From a computer or IPad, go to https://www.rankone.com .
(if using a mobile device use the regular web address not the mobile site for rank 1.)
At the top of the page choose the option for parents and login.
You will need your CBC student ID number.

Select the state from the drop down box (Texas)

1. Select Coastal Bend College from the list
2. A list of required forms will be listed in red, click on a form (one at a time)
3. Enter your student information using your permanent address, phone number, email address and insurance information.
4. When prompted use the mouse to sign your name in the box, if using a mobile device use your finger or stylus.
5. Click the I agree box for electronic signature purposes.
6. Submit form
7. Return to the list and choose your next form
8. Your information should automatically be inserted into the correct places at this point.
9. Continue answering questions and signing documents.
10. Once completed and the Athletic Training Staff approves the forms you will receive an email notification that your forms were accepted.
